Appointment Preparation

To prepare for your initial appointment you will be emailed a form to complete prior to your consultation. If you are unable to complete this form at home, please arrive 10 minutes before your appointment to complete the form in our office.

Please email through any relevant reports from other professionals to [email protected]

If you have NDIS funding, Medicare or Private Health, please advise us prior to your first appointment.

NOTE: We are currently providing services to SELF MANAGED NDIS participants only.
